Our courses are conducted one after another. First courses start in January and every 10 weeks thereafter throughout the year. To apply for a fully funded 10 week course please select the surgery you are registered with from the list below and complete the application form.
Places on each course are limited, so by completing an application, you will join a waiting list and once the deadline for the next course is reached you will be contacted by our health coach, Steve Grantham, if you have been awarded a funded place.
If not we will let you know and automatically put you on the list for the next available course and so on. Your turn will come and we hope we can be there for your Health Coaching needs.
(A referral from your medical professional is required - This could be a text or a paper form).
